1) Report non-responsive SL over general chat and/or TeamSpeak. (This is as much a rule violation as is bunny-hopping or jihad-jeeping)
2) Inform the rest of squad that you're going to start a new squad and ask if they wouldn't mind joining that one.

I see this sometimes but more often it happens from when a SL leaves and the next in line does not figure out he has been "promoted" just yet. I know that has happened to me a couple times. I always avoid the SL position as for some reason the BF2 voip has never worked well for me but TS and Vent are fine.

This has happened to me as well ("Oh sh$t, I'm SL?" after coming back from getting a "drink" or something). There's a number of reasons such as all other squads are full, and we force squads.

Maybe there is a new step 0 in Whiskey's list derived from the original post:

0) Try and communicate with SL to see if they plan on SLing

There really should be an "AFK" flag to set when you gotta get up and take a whiz, get the phone, etc. It already kicks w/o activity after awhile so that should be covered. Maybe after 15 secs or so it could automatically set the flag, and it should put up a special icon next to your name in player lists. It'd have to be a BF2 or POE2 mod thing, though.
