I definitely play better when I'm on my legally prescribed Adderall for my professionally-diagnosed brain differentness. I'm at my worst when I'm coming off it, but my best scoring games were all when I wasn't on anything, maybe one beer.
Oh, um, the thread subject...yeah.
I set up BF2 following Koroush's guidlines and found some to be benificial, others to be completely irrelevant.
What I found effected the entire performance of my computer was following his tweakguide for setting up the OS on a new computer so everything gets installed in the correct order and doesn't have conflicting addresses.
Low settings on textures and shadows--WOW. Even though I played those settings once and noticed the advantage, it never occurred to me to use it to give an advantage. Honestly, I guess I never thought K/D ratio was more important than game immersion!

See, this is what I don't get about tweaking. People dump hundreds, even thousands of dollars into building themselves these uber rigs and then they turn around and tweak the graphics back down to the stone age. My brother does this when he plays games like Quake. It's maddening! It looks like a bloody Nintendo game at no better than 8 bits so he can get the 125 Frames Per Sescond Holy Grail of Quake pwnage. It makes no sense.

Im playing with everthing set to it's highest image quality in game, @ 1600x1200x32x100hz 16xAF/8xQAF. All nVidia optimizations disabled.
It looks awesome and I'd crank the settings in BF2 higher if the option was there. And I'm still over 100fps much of the time.
One thing I hate about the BF2 engine is how grass dissappears at long ranges. People think they are well concealed, when they're actualy in plain view. And the way BF2 draws fog too. It's no huge secret that you can see further by looking 40~ degrees left or right. It like that with both my nvidia and ati cards.
