WHat wiskey said. Turn all other Ingame sounds down and the voice IP will get "louder" in proportion. This also goes when ppl always complain "I canīt hear anything over the Gunship sound!".
There also is a setting of "incoming" transmission. Be sure to pull that up to 100% also.

Since 1.5 I can't hear when people say medic this like medic, supplies etc. Sometimes I hear nothing and sometimes a just a sound which I can't really think of a word to describe.

More bullets on target? More effectiveness at longer range? More bullets landing on certain areas of the body rather than others?

There is prolly 70% Training & knowledge coupled with 30% natural "skill". Meaning age, reflexes, fittness, etc
I think the basics of being a good shooter can be trained fast. But to find out in which situations which skills of you sould be applied is more difficult.
The real secret of a successful shooter is the positioning. As mentioned before taking well thought out cover is THE key in winning engagements even against 2 or 3 people. For your visual pleasure I will show you 2 of the best cover objects on Camp:
Example 1: Dragon Teeth

What is the correct form for longest distance? Jump right before throw? I think I've seen people move forward and jump, but that might just be the visual.

yeah, same as BF2 and itīs mods. Walk forward and jump. Time the Grenade throw to be at the highest velocity.
