What happens to me is I click LMB to drop 1 RDX, then RMB to switch to trigger. The trigger appears in my hand, but when I click LMB again to detonate, instead of detonating it drops another RDX.

Here's the trick. I've been using it for a looooong time and I have never, ever had it fail on me. For reference, MB1 is detonate, MB2 is alt-fire (pretty sure everyone will have this setup, but just-in-case).
Switch to your RDX.
Hit fire until your desired number of RDX have been thrown.
PAUSE for about 2/3 of a second.
Tap MB2, MB1 in rapid succession.
If you click them quick enough, your RDX will detonate even before your trigger shows up. Try it!

I thought it was something to do with not having "finished" taking the detonator out, so when you left click, it decides you still wanted the RDX.
Of course you then have to pick it up or run if you still want to switch and blow it up.

Pause after throwing? Huh, I thought it'd be pause after getting the detonator out to be sure that's what you're using. But i'll be sure to try this, thanks

Is it possible to kill a critically wounded body with your guns by shooting at it? The SOP video on reviving says you can kill someone by rdxing the body...

Oh lyra....
My computer cannot handle much video and audio wise. If I had only one or two settings in the video section to turn up to medium, and one or two in addition to turn up in audio, what should they be?

Not meaning to speak for Lyramion but I'm in the same boat and I put everything on minimum except for view distance.

Not with every gun. I know you can do it with the main-gun of the tank and the walker, Also RDX is working fine. I am not sure about nades.

Splash damage will damage a critically wounded body, meaning explosives (including rockets and grenades) and some vehicle weapons will do the trick. Just regular bullets won't.

The answer is, you KILL people by blasting their body so far away that their kit isn't near their body. Players can only be revived when their kit is nearby. If you think about it, you'll notice a trend that things that seemingly "randomly" kill you are in fact doing so by blasting you so far from your kit that you can't be revived:
Tank blasts
APC mortars
Rorsch shots
RDX demopaks
Gunship missiles
Grenades
Sniper shots

Falling off a building so far that your kit is on the roof and your body is on the ground.
Getting run over by a vehicle and dragged some distance.
No, wait, those are often revivable. I think distance is a good gauge for the real reason but not the determining factor.
