Disclaimer: My knowledge has moon-crater sized holes in it, but just my 2 cents.
Locking squads - Only for specialities such as vehicles, assets or running a mobile squad of under 6 (usually 3) soldiers.
NOTE: More than one or two reduced-number locked squads will make your team less effective. These mobile sqauds are used to supplement the strengths of other, larger squads or to run behind enemy lines. Too many reduces effectiveness of your team as a whole. Usually they will be locked since the SL perhaps wants to try a certain manoeuvre which cannot be explained fully to a group of newcomers who may be uncertain of what to do.
Locking squads may also be acceptable if it is only to first invite some friends to the squad before opening it up to the rest of the team. 1-man locked squads are not acceptable on TG since it defeats the purpose of squadding up. Some IHS's have a slightly tweaked set of these rules, which are still in general-keeping with them, however newcomers to TG do not have the permission to do this.
Naming squads - Fun names are welcome, as long as it's nothing profane, explicit, racist, etc. However it may sometimes be useful to use this as a chance to let the rest of the team know where you're planning to go at the beginning of the round, and perhaps even stay there for the whole of he round, such as naming it after a flag you are going after. Asset squads should be called "Assets" or something similar to signify its role. Vehicle squads should be named after their chosen vehicle.
Speciality squads - Mentioned above.
Vehicle squads will stick to a particular vehicle for most of the game, although they are not necessarily forced to stay in this vehicle the entire round if they change their minds, however in this case, the vehicle squad should then be unlocked if it becomes a general infantry one.
Asset squads will have the aim to destroy enemy Commander assets, while in-keeping with UCB rules of enagagement if this is where they must go. (We need a UCB R.o.E. SOP)
Gunship / tank squads - These vehicle squads, along with walker-dedicated squads should follow the general rules of vehicle squads above.
learn TG squads - Formed to demonstrate to non-TG tagged players who are therefore perhaps unaware of TG, of our gameplay style and beliefs (teamwork, mutual respect, communication, playing for the benefit of the team, etc.)
SLs to teamspeak SL-SL channels - No idea, never used it.
Kit assigning from SL - Realised I'd read it yesterday when I saw the question

Hold spawn - The action of deselecting all spawn points, and only spawning in to the place where your Squad Leader instructs you to, whenever your Squadleader instructs you. Although the tactical advantages of this may not be obvious to some newcomers (it wasn't to me), it's importance is often huge to any strategy.
SL initial fast movement to neutral flag - You obviously had something specific in mind when you put this down Silver, but I can't figure it out. The only thing I can think of is not spawning in on your SL immediately if he is going for a flag close to the frontline immediately on foot. Hold spawn, wait for the SL to say his sprint has run out, then the rest of the squad may spawn in on the Squadleader and use their sprint to further the distance covered by the squad in reduced time.
Mic check in squad - When entering a squad ask for a reading on both your loudness and clearness of speech. (The old SOP was very good on this point so I won't go on, I think that one is more than functional enough)

Locking down for the moment.
Edit: Reopened. We had to decuss a point of asset squads. The prime rule of asset squads is as its always been, First come first serve. We will not kick someone just because they are in your gunner seat. Nor will any TG member greif the player into leaving his spot. That means you will not sit the asset away from action until he leaves. You can ask and tell him what your doing, you may even invite him. But you do not force the player into waiting games. If he was waiting for the asset first and clearly at its spawn point. You must let him take it. There are no holding "E" wars on gunship pads. There will be no reporting of players who are taking your intented asset. There will be no reporting of players that are in a passenger seat making a effort to play by normal means. Of course of the TK you, curse you off, consisently hog the asset without waiting in line over and over, then you may make a report. But these reports must be clear, I dont care he took the asset once and didn't wait.
There is only one exception to this, The CO. The CO may make any orders in the field that fit into his plan and how he wishes to carry it out. The CO can execute any orders that do no violate the server rules such as suicide and RDX jeeps. But the CO can order a squad to use an asset. This will allow the CO direct access to a support feature and route it to where it's needed in support of other infantry only squads. SLs of other squads would be able to call in support of the asset, or directly via TS channels.
Asset squads are not ment for you to lonewolf in a tank every round. They are not ment for you to sit and not think about your team. They are not training wheels for you to learn.
Asset squads on large vehicle maps/Titan/NS are out of place and hinder teamwork more then it helps. As all squads need to deal with armor most of the round. Tanks, APCs, Walkers, FAVs, and Transports are never to reserved on these maps. The first come first server rule still stands in all vehicle cases. The only exception via CO order is Gunship asset squads. Tranports are included in this rule, While you are still allowed to run gunboat platform, the main used of this is to transport squads, This asset should be open to all SLs at any time.
Asset destruction squads do not reserve any vehicle based assets and do not fall under this rule. They are free to destroy enemy CO assets as long as it fits in your CO's plan.
Asset squad names shall only be: Tank, Walker, APC, Gunship(s). This is to be clear to all SLs, CO, and other teammates. FLYSHIP, FLYBOYS, ROFLSHIP, ROLFAPC, ARMOR, etc are confusing at times and some people miss the meaning of your squad. Clear worded squad names show clear intent and more serisous demeaner for teamwork. Lets not forget to show these public players how it's done. Lets try and stay away from ARMOR itself. While a entire asset squad holding all asssets on a map might work, it does shoot out a elitist persona to people who don't know TG. ARMOR itself should only be reserved for when the CO requests it and fits it into his combat plan or your SL seriuosly thinks about the teams need for an entire asset squad. If the SL decides that it is needed, the squad shall allow any requesting members to join the remaining slots and rotate out drivers and have the other drivers in support of the armor. If there are no requests, then the squad may function as the SL sees fit.
Everything here is a guide on how to make asset squads. They are not law nor rule. The only rule is first come first serve, and the CO exception.
