CRASH on Titan map changes

I need some help. I will start this by saying I can play conquest all day with out any crashes to desktop. At the end of a titan round while the next map is loading I crash to desktop. Im not talkin every once in a while but EVERY time. So I come back re-load with no problems, play the round and crash at the next map, rinse and repete. Here is what I know so far.
It is not tied to just our titan server, same results on 4 different titan servers while testing.
I have gone as far as to uninstall-reinstall BF2142 and 1.25 then NS then 1.25 again and same problem prevails.
I used to be able to play titan all the time.
I have not changed any hardware or software on my system.

My specs:

6600 2.4ghz dual core
2 gigs RAM
256k 7900GT vid card
VISTA 32 bit
running in admin mode
updated vid drivers, was able to play after this change

Its not a totally horrible problem, very annoying having to restart 2142 every round but I would really like to work around this. Any suggestions would be appreciated.

I had a lot of rpoblems with Vista and Titan. I would CTD amd even more common is the titans would lose their skins and beccome completely black.

UPDATE:
I did the fix that helix has in his post and it worked wonders. I have been able to play 3 test rounds so far on a titan server with 3 map changes and 0 crashes, wonderfull stuff!! Major Props to helix on the find!! Thanks again Helix, you da man!

Yeah, I had to do this a while ago when the new patch came out. Every second map would cause me to crash and end of titan (or sometimes entering the titan) would cause a crash.

Mach owner was having some titan issues on his XP system and ran this same program, I will get him to post if he has seen any changes. Playing Pfrosty Pfriday on friday was awesome, no CTD's at all. I highly recommend the program to anyone who has CTD problems, I think it helps 2142 all together not just titan issues.

I tried this but I'm still crashing on titan.
