Re: Knifing... again.
In the situation I described, I wouldn't have gone for the head shot. The USP has a pretty rapid ROF, and the chest is bigger than the head (one hopes). I'd have unloaded the entire clip..... quickly.

Isn't it great that knife now does double damage to the back? I remember back in the day when you would have to knife someone twice just to kill. Just to remind people who think that nothing good came after 1.5.

Re: Knifing... again.
Really? I'm interested to hear your reasoning behind that. The glock can be very deadly at point blank on burst fire, but I think the USP has better damage vs ROF. I'm still fond of the 228, but then any pistol that I can comfortably snipe with is a pretty handy weapon.

Exclude Five-seveN I forgot it can hit from a distant but it isn't as good as the others

Re: Knifing... again.
In that situation (behind 5 unsuspecting enemies, not that it has EVER happened to me, but for the sake of discussion), I think I would trail them until they split up, making it more likely for me to kill them in smaller groups than all at once.
Once you've broken them up (2 and 3, 4 and 1, 2 and 2 and 1, 3 and 1 and 1, etc.), the weapon you chose to kill them with does not become such a big issue.
However, if you were absolutely forced to engage all 5 of them right then and there, I would say start by knifing the last guy, and then pull out a gun when you have to (preferably when there are only 2 left).
Or an MG might do the trick of they are all bunched together. I would strongly advise against flashing a group of 5 enemies from behind, because you've just put a timer on how long you have to kill them. Although flashing does disorient and temporarily blind, it would take a great marksman with incredible fire discipline to a)headshot everyone in 3-4 seconds, or b) kill 3-4 with one magazine and then reload to finisht them off.
However, if you can manage to get them down to a group of 2-3 (either by patient following or stealthy knifing), a flash would be a gret idea before finishing them off.
