I know for the achievement you can't so much as pick up a primary weapon. That's about all I know though.

not true, as long as you do not do any damage with any weapon other than pistol or double pistol.

Quite possibly you used a med-pack or pills, the game auto-switched to your primary, and you used it accidentally.

Okay, did akimbo, you can at least use pills and the game is fine with others healing you. Although thinking about it now, being able to heal yourself makes sense too otherwise you would be forced to earn Unbreakable and Akimbo Assassin at the same time.

Quick FYI for anyone trying to get all of the achievements... the game lets you earn them on easy, I ran No Mercy on easy and came away with: Do not disturb, Akimbo Assassin, Stand Tall, and No 1 left behind.
