I'm not sure how you can accidentally shoot yourself in the head while attempting to shoot at someone else, but I'll go along with what the PD is saying...

Maybe running with the gun in his right hand (for a righty) and trying to shoot over his right shoulder when the gun jogs suddenly to the left - after a shot is fired, e.g.?

I am not sure of the right vs. left handedness but this is what the PD said. Apparently he was trying to flee and shooting over his head (behind him).
