I thought it was just a medic bonesawing a spy. I think the whole heal then bonesaw is rather silly. Of course they did make you carry a garden gnome around for EP 2.
I think it'd be cool if Valve opened it up, or we found a way, to make our own achievements to be added. Granted they wouldn't be able to be displayed on our steam profiles but still it would be nice to add Lightning Offense for Castle
